LOWVILLE — The Lewis County Hospital Foundation is seeking nominations for its third annual Citizen of the Year award.

Nominees must be current Lewis County residents who have a strong record of community involvement and service, with a special emphasis on volunteerism. They may be entered as either individuals or couples.

A subcommittee of the hospital foundation board will select the 2012 recipient or recipients.

Nominations will be accepted through July 20. They may be sent by mail to Lewis County Hospital Foundation, 7785 N. State St., Lowville, N.Y. 13367 or by fax to 376-8216.

Nomination forms are available at the Lewis County Chamber of Commerce and Journal and Republican offices on South State Street and at the Lewis County General Hospital switchboard.
