State police charged James Jarvis, 29, Black River, on Jan. 23 with second-degree forgery, a class D felony.
Mr. Jarvis is accused of forging a prescription for tramadol by altering the number of tablets for an additional 50 tablets when he requested it be filled at the pharmacy in the Walmart Supercenter on Route 11 in the town of LeRay.
He was arraigned LeRay Town Court and sent to the Jefferson-Metro Public Safety Building on $500 bail.
