Former Mercy Primary Care Clinic nurse practitioner Kevin F. Brennan surrendered his license after being investigated for misconduct.
According to the state Education Departments Office of the Professions, Mr. Brennan did not contest the charge of forging prescriptions for his own use. He was arrested last month by state police on a Class E felony charge of first-degree falsifying business records, accused of making a false entry on a prescription form for 48 Percocet while acting as a nurse practitioner for the clinic.
Mr. Brennan, 56, also was cited with professional medical misconduct in April 2012 when he agreed to a two-year stayed supervision, two years probation and a $1,000 fine. He did not contest then a charge of having been found guilty of improper professional practice or professional misconduct in Alaska.
